This ECHO is by invite only as part of a pilot research project. If your clinic has agreed to participate in this project, please register for this program.
This 7-session ECHO series is designed to help primary care clinicians and their team members: 1) support patient-centered healthcare planning and decision making by people living with dementia (PWD) and their family care partners and 2) implement and track ACP for PWD in your practice.
Topics will include:
*Accreditation: Oregon Health & Science University School of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
Credit: OHSU School of Medicine designates this live activity for a maximum of 7.0 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
