Partnering with CHWs: Diabetes and Hypertension Prevention and Management

This 12-session ECHO series will cover topics such as:

  • CHW scope of services
  • CHW roles and responsibilities to address chronic disease
  • culturally specific services and accessible services
  • nutrition education that is culturally specific and addresses food insecurity
  • effective community health worker supervision strategies

We are seeking a diverse group of 40 participants who are interested in leveraging the expertise and partnering with community health workers in their organizations which may include, clinicians, clinic managers, community health worker supervisors and others.

*Accreditation: Oregon Health & Science University School of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
